Can anyone share your plan design for a dependent Spouse Voluntary Life insurance plan that is not a flat amount rate. We are trying to set up a Spouse life $10k, $25k, and $50k with age reductions based on spouse age. The Spouse voluntary life insurance rate should be based on the reduced coverage amount (per 1,000).
