BN297 EIN Population

I have been testing out how the BN297 format populates on our 1095C form with our MHC print partner and the data looks correct except for that the fact that the EIN number is not populating in the csv file at all. I was under the impression that the EIN would pull directly from the EIN I entered in the BN00.07 to roll everything up into our main company - is my understanding not correct or is anyone else seeing this issue?

On a side note, I am also running into an issue with the printing not resolving on the page properly - the perforation cuts it off in the wrong place. I have already opened a ticket with MHC but anyone else using them for printing having this same issue?
