Form auditing for SOX compliance

Our System Administrator is working with some SOX auditors and we need some help.

I know that there is a "Forms" table in the "Forms" database. 

However how can I tell what the "modified date" and "modified by" are when I apply Form changes from my "Test" Syteline to my "LIVE" Syteline system.

The Forms table doesn't maintain any "modified by" information. As far as I am aware you're only options here would be to either use a query to determine the last time the table was modified, which probably won't get you what you want. The other option would be to use source control with your internal dev environment. The check out/check in process will record who and when and allow you to do some other life-cycle management.

Plus, if you use source control with form control you'll have a full audit trail for edited forms as well as other global objects.
