Adding a spousal surcharge to the health plan

We are adding a spousal surcharge that employees need to pay in addition to their health insurance coverage amount. What is the best practice on setting up this deduction. We have considered adding another option to the existing health plan with the surcharge amount added to the existing health insurance deduction or should we create a new health plan for only the spousal surcharge. The employee would be enrolled in the spousal surchage health plan and the existing health plan. Two deduction codes would be used in the last option. How are other companies handling a health insurance spousal surcharge. Thanks for any information.

We also have a spousal surchange and for 2016 we are moving it to it's own plan with a new deduction code. Currently it is built into the medical premium along with a tobacco surcharge and wellness incentive. Moving it out to it's own benefit and deduction will make things easier all around for us.

Is your deduction code pre-tax or after-tax for the surcharge.
