BN729 showing $0.00 cost

We have applied every ACA-related CTP to date but the BN729 is still creating records with a $0 employee cost for 2015. When I run BN729 for 2016, the employee cost shows up. I get the same results when I enroll an employee via BN31.2 - 0 cost on BN29 for 2015 but the 2016 record shows a cost. Is anyone else still having this issue?
The offer & safe harbor codes are set up on the Plan. For the groups that don't have the best offer, we've set up overrides for the offer codes on the contribution forms. The only BN729 records with an employee cost is one of the override groups who are qualifying for coverage because they measured at 30 hours or more during the measurement period; if not for ACA they would not be eligible for coverage.
Any suggestions on what I should look at/change to get the employee costs to display correctly?

The following change is included in the CTP:
1. Corrected the creation of offers from BN31.1 when all offers being created have plan Contribution setup reflects all Coverage options available. This will facilitate correct reporting of the employee cost instead of 0.00 on BN29 and BN729. (JT-874674)

Tina - yes. We applied it last night and I reran the BN729 with my fingers crossed that it would work but no such luck.

Check the Contribution record set up. Any setup for the Contribution's Options that are Active but with no cost = 0.00. And likely why the Bn729 is reporting lowest cost of 0.00.
Example
Coverage Options setup for 1,2,3, and 4
Contributions employee cost setup for options 1 = $100.00, 2 $200.00, 3 $300.00, 4 no cost.
Bn729 will see that Active option 4 and report 0.00 as the lowest cost.

Thanks - unfortunately, none of our plans have a $0 cost.
I'm wondering if I just need to override the employee cost on the ACA screen for each of the contributions groups. Has anyone done this?

Posting an update in case this helps others. Issue was resolved after reviewing the BN215 contribution setup. There were contributions for active options and blank contributions (translated to 0.00 cost). Those blank contributions were set to Inactive and then when the BN729 was run the true lowest rate came through.
