Performance Appraisal - self appraisal

I want to add a self appraisal section that asks questions for the employee to complete. I cannot figure which section type would accomplish this. It would be the only section that is for 'self'.

Example:

Name three accomplishments in the last 12 months. Why do you think those were successful?
Provide an example of a tough situation you handled well in the past year, and the key strengths you relied on.
Give one or two examples of how you have grown professionally over the last period.

One way to accomplish this is to engage the check in process and let the employee answer those questions and then on the form pull that information into it.

We are just needing this for a few select employees for appraisals that are coming up quickly. Is there another option besides having to set up check ins since we do not currently have that set up. Thanks.

You can create User Defined Criteria with Long Text answers. Put them in a Criteria Group and then attach the Group to the Appraisal.

That worked nicely. But I get an error that is not allowing the employee to submit the self appraisal: "Cannot submit appraisal; overall rating required in summary section". The appraisal does have criteria groups that are rated but not the Criteria Group that is the self appraisal. Is there a setting I'm missing so this will work?
