Identifying Individuals with disabilities for AAPs

We are a federal contractor (healthcare) that must comply with the new regulations to include an Affirmative Action Plan for Individuals with Disabilities (IWD) beginning our next plan year. Up to now, we have not captured disability data. We plan to use the Disability flag on HR11 to capture a Y or N but are looking for creative ways to survey existing employees and allow them to declare one way or the other via ESS. NOTE: we will also push out the Veteran Status survey at the same time to allow existing employees to (re)declare veteran status.
Has anyone pushed out a disability survey via ESS to comply with this new requirement?
