IRS 941 5d Fica Med for over $200,000 Wage Earners

As I am new to this what do you use to compute Column 1 of 5d on the IRS 941? IF I use PR276 to get the EE Medicare Wages do I include all wages or just those wages in quarter 3?

Let me ask the question in a different way. I now found PR287, any ideas why a company would not utlize the PR287 to produce the 941? Are there issues with the program? I noticed when I run it without tax liability by day it completes in a couple of minutes. With that turned on is taking a long time to complete. Is that normal?
