PA49.1 Employee Tests add-ins template?

Has anyone successfully used an add-ins upload for PA49 Employee Tests? If so, would you be willing to share your template? I did this two years ago, but something must have changed because I can't make it work now. Probably hidden fields or something, who knows. Thanks.

There are sample templates for PA49.1 and PA43.3 on Infor Extreme. If you click Downloads, Products, Technology, Technology Sample Content there is a zip file with hundred's of templates. The ones at the bottom are the excel Add Ins the ones at the top I believe are for Spreadsheet Designer.

Thank you all! Trying to mass add results (so they are different for everyone). I will look for the templates on Infor Xtreme, and Margie thank you for those specifics.
