ESS Open Enrollment: spouse tobacco and employer provided insurance attestation

We have changes to our benefits plans this year that will require when an employee enrolls in spouse coverage, they will be required to attest to the spouses use of tobacco and whether or not the spouse is being offered benefit coverage via the spouse's employer. The results of this attestation will effect the employee's premiums.
We are interested in knowing how other companies are handling this via ESS Open Enrollment if at all.

We are actually requiring the spouses to come to open enrollment, blood draw (to check for tobacco use), blood pressure, height, weight, all that stuff....
We have a wellness program where if they are covered under this companies insurance, we get cheaper rates if certain criteria are met.

We set up health plans so that there are several coverage options (BN17.1). For example, option 6 is "EE+1 no tobacco+no spouse ins", option 7 is "EE+1 no tobacco + spouse ins", etc. Each coverage option is associated with a premium rate (BN19.1). During open enrollment, employees chose the appropriate ooverage option but must also sign an affidavit for spousal insurance coverage and an affidavit for tobacco use status. After open enrollment, HR manually checks that they have received affidavits from all those claiming such. If not received, the coverage option is changed accordingly.
