ACA Reporting when Different Plans are offered

We have two plans, one is a PPO and the other a CDHP, with different employee costs. The CDHP is the cheaper cost plan. Both meet the qualification of qualified offers, which amount do I report on the 1095c ?

Uploaded my offers using BN529, but the waive plan is not showing in BN29. Is this working as designed? In Test it showed for me, but not in Production.
