Custom forms with workflows shows blank task

Hello,

We have created a custom drill back for OIS120 (M3 Program) and used it in workflow. But when we try and use it as a custom form in the task, the task is blank with just done button. I tested the drill back and it is working fine.

Is there a way to make this form work? Am I missing something?

Something must be wrong with how you specified the custom form in the workflow model. More information (like screenshots of the setup) is needed to answer this.
