This is caused by one of the pre-requisites not being installed.
I can't remember exactly which one right now but I think it is one of the .NET requirements.
What operating system are you installing on?
you got this error regarding Bootstrap User probably because the SAML session provider wasn't installed correctly in the installation process.
Why you're trying to install version 12.0.37 and not 12.0.40? You have Windows Server 2016 which is still supported by version 12.0.40.
I think it's probably 1of these 2 you are missing..NET Extensibility 4.6ASP.NET 4.6

I was typing my reply but it disappears. Anyway. I think you try to install it on a windows server 2019 the latest version of Infor OS 12.0.40?
That will not work. I've had the same issue, when you install it by using AD FS or Infor STS, that will not make any sense. When you click OK the installer will go on, but on the end you're missing your grid part which you have to download from the OS Manager. Even creating the bootstrap user manually will not make any sense.
This issue I had at a customer in Norway and created an incident (14326758) in February 2020....! And Infor can't give me a solution. They are still working on it. In the meantime the customer provided me a server with windows server 2016 which is working without any issues.

I am trying to install InforOS ver 12.0.37 on windows server 2016 standard.
ADFS Server details: ADFS installed on windows server 2012 R2 standard.
SQL Server details: SQL Sever 2017/18 installed on windows server 2016 standard.
Please advise on this.

I've got the same error message when I was installing InforOS with a local user instead of a domain one. Also, the same domain use has to be specified as the ION user.

Is there a way for you to install version 12.0.40 and have the ADFS server installed on Windows 2016/2019?
The ADFS server you have today (SQL-SRV) which is running on Windows 2012, has good performance? because the first error you got was "connect timed out" - although it's not stated to where it tried to connect.
How much memory does your Infor OS server have?
