Images in a RichText field

We have a user field defined as RichText and I'm trying to insert an image.  The Insert Image icon lists a default of /images/placeholder-80x80.png.  I tried using an image stored in the UserImage business class (I don't know where /images/ is). I've also tried using the img tag in the html. On the save or when the VISUAL button is selected on the RichText field the image is removed.  Any ideas would be appreciated.  BTW, this was working until the October CU I believe.

Here's how we have pulled images from the UserImage business class onto forms.

Method 1 - Create a Relation and Place the Relation Directly on Form

1. Create a Business Set relation between the business class you're pulling the image into and the UserImage business class. To create the relation we created a DerivedField on the UserImage business class to return the value of "HROrganization" and then used this DerivedField to help create the relation. 

2. When you create the set you will need to reference the name of the image file you uploaded to UserImage.

The LPL would look something like this:

EmployeeToUserImageCompanyLogoRel is a UserImage set

     Instance Selection

          where (Employee.HROrganization = related.DerivedHROrganization and related.UserImage = "CompanyLogo")

3. On the form you can enter the relation

   Layout

         single column

              Employee

              Employee.Job.JobLevel

              WorkLocation

         Employee.EmployeeToUserImageCompanyLogoRel.UserImage

               display as full image up to 11 lines

     DerivedCompanyLogo is a DerivedField

          type is BinaryObject  

          return Employee.EmployeeToUserImageCompanyLogoRel.UserImage

4. Place the BinaryObject DerivedField on the form

 Layout

         single column

              Employee

              Employee.Job.JobLevel

              WorkLocation

              DerivedCompanyLogo

                    display as full image up to 11 lines
