Self Identification Components Do Not Display in EEO form

Hello,

Can anyone tell me if I am missing a setup for EEO self id components to display in the form? I have marital status, birthdate, gender and ethnicity for self id components.

And yet when I look at my EEO preboard and onboard forms, only gender and ethnicity shows.

Am I missing anything. I have attached the setup.

Not sure if you opened a case with Infor yet, but a few things to look at:

1. Do you have more than one 'Self Identification Configuration' built and if so, what one is tied as your "Requisition Default " under Talent > Organization.

2. If you have multiple, which one is tied to the Requisition you are applying for?

3. If you do a CTRL+SHIFT+Left Click on the application by the Equal Employment Opportunity, what Form is referenced? Example:
