Activation Policy isn't triggering Workflow

I built a custom BOD and an Activation Policy to trigger off of that custom BOD. The Activation Policy will then kick off a Workflow. For some reason, I can't get the Activation Policy to trigger when the BOD comes into ION. Would anyone know what is the problem with my Activation Policy or custom BOD? Please see attachment for more details.

Thanks Keith for all the help with this. I finally got the Activation Policy to trigger from my Custom BOD. The issue appears to be caused from the Control Properties setting in Partner Admin. For the Agreement Group, it was initially set to ionToLogicalId and once I changed it to ionFromLogicalId. And then did a Reload in LCM, it started working. See attached screenshot.

Could you also post a screenshot of the "Rule" tab of the AP? Can you test with just one of the conditions triggering the WF instead of both?

I tried to just trigger off of one rule. I tried just the AccountingEntity and then tried the ActionCode. But it still didn't trigger.
