Split Requisitions

I had calls from two requesters in the past couple of weeks wondering why they only received a fraction of what they requested through RQC. I had both of them send me their printed requisition. When I looked at their requisitions in RQ10, I noticed that only some of the items on the printed requisition appeared on the actual RQ10 document. As I dug a bit further, I found another requisition for the requester that was unreleased which contained all the items that were missing. The document number for the unreleased requisition was one number off from the printed req (ie: 241163 released, 241162 unreleased).
Both requesters have been using RQC successfully for quite awhile, so I don't believe it's a user issue.
Has anyone else seen anything like this?

Yes, we've run into it a couple of times with different requesters. It's happened to three people that I know of, two of which are power users ordering every day, making it unlikely to be user error. Haven't been able to duplicate it myself yet though.
