potential issue with core saleslogix save() ?

We ve been getting the below error whenever problematically saving changes to Account or Contact objects using Account.Save() or Contact.save() in the OnAfterUpdate hook.

Row was updated or deleted by another transaction (or unsaved-value mapping was incorrect): [Sage.SalesLogix.Entities.Account#A6UJ9A0016W7]

The code doing the Account.save() worked fine in SLX754 but suddently slx 8.1 fails to save using Account.save()
Unless anything else changed, how should we go about updating the Account or contact objects in business rules ?
