FSM - Epic Item feed (via Clinical Bridge) - Epic needs cost per usage.

Hi all, We are using FSM - Epic Item feed via Clinical Bridge (CB). The unit cost CB uses comes for the contract line Base Cost For Lowest Divisible Cost. Epic need the Usage cost which we do not have in FSM. I believe Infor has a new field (JR-3523766) now. Has anyone (Hospitals) implemented this or are using a workaround to send usage cost ?

A few initial thoughts in hopes this may help:

- JR-3523766 added the ability to specify a UOM Code (e.g., EA, PR, RL, etc.) on the Contract Line that corresponds to the Base Cost For Lowest Divisible Cost. If you do not specify a low UOM Code, my understanding is that Epic will automatically interpret the UOM for the "Base Cost for Lowest Divisible Cost" as EA.

- I'm assuming you're talking about Contract Special lines that are not in the Item Master. If they're Contract Lines that are also ItemMast type items, then I believe the UOM conversion and Low UOM Code (called Stock UOM on the ItemMast) would be derived from the ItemMast record and the Base Cost for Lowest Divisible Cost on the Contract Line would be calculated accordingly.

- Ideally, your Base Cost for Lowest Divisible Cost and the UOM Code corresponding to it would represent the Unit of Use and per unit Cost (what you call "usage cost") for your clinicians in the EMR. So, if your Contract Line has a higher Base UOM such as CA or BX, you'll want to make sure the UOM Conversion gets you to that Unit of Use by which your clinicians will be charting for that item.

- Example: If you have a contract line for Sterile Lap Sponges at $1/PK (with 5EA / Sterile PK), but your clinicians actually count, chart, and bill your Lap Sponges in the EMR by the EA, then you would want to ensure your UOM Conversion is 5 and your Low UOM Code is EA, with the calculation for Base Cost for Lowest Divisible Cost thus equaling $0.20. If your contract line for the same item is $40/CA (with EA 5 / PK 40 / CA), then you would want to ensure your UOM Conversion is 200 and Low UOM Code EA, with the calculation for Base Cost for Lowest Divisible Cost thus equaling $0.20.

- Unfortunately, at this time we don't have the ability at the Contract Line to specify the full packaging string with UOM conversions if you had your Contract Line at the CA (as in the example above) and wanted to show conversion to both PK and EA.

Thank Ryan, these are non-special / Itemast Items which do not show this field on the contract line. Also they wanted to specify usage (nbr of time/sq inch, etc) which was not possible in FSM as they did not want to setup / show these to the users. Epic figured it out on their side so we are good.

Thanks Ben. Glad to hear you figured it out. Depending on the Item, it can be difficult to represent that Unit of Use as you have found, especially if you want to suppress it from certain users. The Item also has a field called "Quantity of Stock UOM." This is intended to suggest how the Stock UOM (which will ideally be the lowest possible UOM in your Item UOM table) can be broken down further. This "Quantity of Stock UOM" is picked up by the standard ClinicalBridge Extract list for FSM, but you'd have to see how your specific CB installation handles this field.
