Pvx Web server with FACTS 7.5

Looking for an example lib/_web/START_UP.MOD that would work for using the Providex web server with FACTS 7.5. Getting error 13 in SMU955 and have followed the infor365 knowledgebase instructions of creating a copy of the START_UP.MOD with a lower case name... error persists.
