Unit of Measures EA but there are 5 in the each

Medtronic has changed their unit of measure from a pk of 5 to an each of 5 for an item. Is anyone experiencing this issue? How are you handling it? We interface with the OR systems and utilize the conversion factors to bring down to the each cost for patient charging.

We've had this problem too. The resolution was to build IC11 as a PK of 1, EA of 5. Stock UOM would be PK

This allows us to order with the correct vendor UOM, but provides the conversion factor needed for the Patient charging system.

We've added IND-Individual as a UOM - IND of 1, 5 IND/EA. We also have USE for Da Vinci type items that are 10 USE/EA.

The DaVinci robot supplies have this too- we typically ignore the "smaller than an EA" but if it's patient chargeable we use PU (Patient Use) as the stock UOM so we can use EA with a conversion factor of 5.

We use one of the numeric user fields in IC11 to enter an additional conversion factor to break the cost down to the unit of use when needed (if the field is blank then = 1). Robotics supplies and mesh are good examples.

At what point do we all push back on vendors to make appropriate use of meaningful units of measure? All these workarounds are so frustrating and a waste of our time collectively.

I do appreciate all of your solutions though in the mean time.

I've struggled with the uses part of this as well. Would like a solution that follows ANSI X-12, but can't seem to find one. We place the number of uses in the 2nd description field and let our EMR team know when changes occur.

Hi Gretchen -

The problem with the robotic supplies is that they ARE an EA. They just have a limited reuse. Typically, we wouldn't care, but they are contributing to the case charge per use - so it's mostly our Finance Department that wants to break them down.

The Mesh is similar - the vendors ARE selling us an EA. We then tear them apart to track the reimbursement and implant - causing the issue.

It only comes up with the high dollar and potentially reimbursable items - but Corporate Finance sometimes likes to know the reuse factor on trays etc. We just don't break it apart in our item master for them unless it's actual patient care. We used to use a user field like Jon - we changed when the item master needed to start feeding the OR billing programs.

Kat - I noticed that it seemed some of the responses were not necessarily the same scenario. We also have the robotic multiple use items, but I was referring to the single use items Yvonne was asking about. That to me is the more frustrating issue.
