Engineering Change Request Form

Hello everyone,

I am wondering if anyone would be interested in sharing their printable ECN request form template for use with Visual ECN? Or point me to where I can find one.

My co-workers want me to be the initiator to create ECN's but I need a form for them to fill out that helps me fill in all of the header fields as I create the ECN for them. Anything at all would be very helpful. I just don't want to recreate the wheel if I don't need to.

Unfortunately i do not have an answer for you, but I too am looking for a similar form or template to create at ECN at our office. Hopefully someone out here may be able to help share a form that they are using.
